Why have I received this Verification Pack?
ProSearch have been appointed by the company in which you hold an investment. The company is concerned that you have not received all of the benefits due to you as a result of your investment in their company.
The reasons that the company has asked ProSearch to contact you are outlined in the Verification Pack that you have received. This can mean that you have not cashed a number of dividend payments or that you have been recorded as "gone-away" on their register of members , or as a result of a take-over or merger, you have not received the consideration paid when your shares were acquired under the terms of an offer or merger.
What do I do now?
The Verification Form is designed to make the recovery of your entitlement as easy as possible. Simply follow the instructions and read the guidance section detailed within the pack and return a completed form (along with any necessary documents) to us as early as possible.
The person named on the form is deceased - what do I do?
If you are the executor(s) or administrator(s) of the estate of the holder, you should complete the Verification Form and return this along with a Court Sealed copy of the Grant of Representation. The Verification Form should be signed by all named representatives. If a named legal representative has passed away, please enclose an original or certified copy of the Death Certificate.
If a Grant of Representation was not required, then please write to us enclosing an original or certified copy of the Death Certificate of the shareholder so that we can assess how best to proceed.
